If you would like to request any additional Section 5 plates, please complete either the Application for Additional Repair, Farm, Transporter, and Owner Contractor Plates or the Application for Additional Dealer Plates

Please Note: During your renewal period, requests for additional plates will not be accepted. Please apply once the renewal period has ended.
